Persons interested in admission to the Radiation Therapy Technology program are strongly encouraged to use the links available on this page to obtain valuable information as well as the forms needed to complete the application process. Application procedures, deadlines, and admission requirements may be found in the Radiation Therapy Technology Program Brochure. Application procedures include:
- Complete a Chattanooga State application form.
- Complete a Nursing and Allied Health Division application form.
- Submit an official high school transcripts.
- Submit an official transcript from any post-secondary institutions.
- Submit official college and Radiologic Technology program transcripts.
- Submit ARRT score report (or provide scheduled test date).
- Document 24 hours of clinical observation by submitted the clinical observation form.
- Submit the clinical observation evaluation form.
- Submit the following three reference forms: Clinical Instructor Reference, Personal Reference, and Program Director Reference.
