Physical Therapist Assistant Program

In addition to admission to the college, prospective students must also follow the steps to apply for admission to the Physical Therapist Assistant program:

1. Submit an Allied Health Application form
2. Submit official transcripts from all colleges previously attended to the Allied Health Applications Coordinator. Please do not ask the Chattanooga State Admission Office to forward a copy of your transcript to Allied Health. (Transcript Request form)
3. Take the HOBET. (HOBET Registration form)
4. Complete 16 hours of clinical observation. (Clinical Observation form)
5. Submit two recommendations. (Recommendation form)

The deadline for priority consideration for this program is the first Monday in March of each year. For other information contact the Allied Health Division office (423) 697-4450.
