  2. Provide official transcripts to the Admissions Office and to the Allied Health Application Coordinator. (Transcript request form)
  3. Take the ACT, SAT, or COMPASS, if applicable.

This form is available at the Nursing/Allied Health Division Office (Albright Omniplex, room O-92), by mail or by phoning (423) 697-4450, and online. Directions for submission are included on the form. The deadline for receiving your application to include you in the selection process is May 15.
